What to Bring to the Appointment

• Patient's insurance card
• A list of current prescription or over-the-counter medications, including dose and frequency
• Pertinent information about your medical and surgical history
• Previous urologic evaluations and/or other relevant medical records
• Physician referral forms if required by insurance

Scottsbluff Urology Associates is a provider for Medicare, Blue Cross Blue Shield, Midlands Choice, Mutual of Omaha, and other major insurances. We will file claims for all insurance companies. Particular coverage decisions are between you and your insurance provider.
